how to get a beautiful green doughnut
The guidance below is intended to help you to maximise learning opportunities and document them with the greatest efficiency.
what are the new assessments?
There are a variety of new assessments, there is an entirely enjoyable document about them which can be found here: https://rcoa.ac.uk/documents/2021-curriculum-assessment-guidance/introduction-0 .
For those who do not want to read the whole thing; these are the important highlights.
SLEs
The most important aspect of assessments to take on board is that they are intended to be documentation of the learning journey. All events are recorded as a Supervised Learning Event. They document a reflective discussion between learner and supervisor. As part of the SLE your assessor puts a supervision level which is not the supervision just received, but the supervision required for the next time the event (case / procedure / list) is repeated. So a novice will have SLEs for similar cases where the supervision level changes from 1 to 2a to 2b, over a period of time. A senior ST will do a complex case with supervision and move to not requiring supervision on site, towards the end of their SIA.
HALOs
Higher Learning Outcomes are intended to be signed off towards the end of a Stage of training, not as you go along. If a HALO is completed half way through a Stage then further learning within that domain cannot be demonstrated. We have moved from asking trainees to get CUTs signed off as they go along, to asking you not to sign off HALOs until you have nearly completed a Stage. Trainees who do have HALOs signed off early may be asked to rescind them.
However, it is enormously helpful both to both trainee and trainers for the trainee to create the HALO as soon as one SLE has been mapped to it. This is because the supervision level will then display for each SLE, making progression clear, and also the SLEs get mapped to the individual key capabilities, allowing everyone to see that there is something in each section. A HALO cannot be signed off if there is a KC with no SLE in it.
The good news:
A-CEX, DOPS, CBD and ALMAT assessments are all remaining, badged as SLEs, but have been updated to prompt the assessor to give the level of supervision the trainee requires on a scale of 1-4. With 1 being direct supervision with supervisor in theatre throughout, up to 4 which means manages independently, with no direct supervisor involvement.
The MSF remains the same.
New assessments:
A-QIPAT is a new assessment for assessment of QIP/audit project involvement.
MTR (multiple trainer reports) are a new requirement at many important progression points (IAC/IACOA/HALO fulfilment), they allow collation of consultant feedback. MTR are triggered by college tutors/educational supervisors as required. They need at least 3 responses to be considered valid. MTRs are now mandatory for HALO sign off and will be checked at ARCP.
What are EPA?
These are ‘Entrustable Professional Activities’ which are now used as assessments for the IAC and IACOA, which are EPAs 1 and 2, then 3 and 4 respectively. Each EPA represent a clinical activity with an associated level of independence needed to be gained. These replace lists of previous CBD/DOPS/CEX needed. Please see more details and links to workbooks on the Stage 1 page, which is the only stage this is relevant to.
what do i need for halo sign off?
There has been confusion about what is required for HALO sign off, particularly as AiT move around different departments. There is a guide available for each HALO, and each stage of training. This is a guide, not mandated, but aims to encourage consistency across the Deanery.
Please click each HALO name to obtain the guidance (still being finalised so not all have links)
